The 7 inch by 5 inch bronze plaque with a black background and brass scres for mounting. Please provide the year your home was constructed, the architect, builder or 1st homeowner. (If uncertain about year or name, please call the Lakewood Building Department at 529-6270 or the Cuyahoga County Auditor at 216-443-7010. You must visit both department in person.)

Price includes a $20 tax deductible donation for each plaque ordered.
